When it comes to movie villains, it is the highly intelligent characters who are often the scariest. The "Saw" franchise introduced moviegoers to John Kramer, also known as the Jigsaw Killer, or just "Jigsaw."
The Jigsaw character very methodically puts his victims through sinister tests in order to teach them an appreciation for life. What is ironic about Jigsaw is that he seems to have almost "good" intentions, though he has very twisted ways of demonstrating this.
Of all the modern movie villains, Jigsaw definitely ranks high when it comes to the "ick" factor of films. When evaluating movie villains, a good test is usually the number of people who cover their faces in certain scenes.

No. 1: Hannibal Lecter
Few movie villains make people grimace and tremble like Hannibal Lecter in the film "Silence of the Lambs."
Hannibal is another example of a brilliant and methodical mind with a very creepy dark side. Some movie villains are sufficiently fictional and far-fetched enough that audiences can suspend their disbelief in order to deal with the horror. With Hannibal, people can feel that he is a bit too real for comfort.
In addition, there is the fact that Lecter is a cannibal. This makes him sufficiently evil in the eyes of most watchers because most individuals are unable to digest (no pun intended) the idea of one person eating someone else.
Needless to say, Hannibal ranks very high on the list of evil movie villains that truly make people shudder.
